I couldn't listen to the audio, but when JDAMs hit friendly positions, its almost always because the TACP erroneously relayed his own posit instead of the target's posit. There's little or nothing a pilot can do to prevent such an error if the TACP doesn't catch it on his own during the 9-line or the readback.
To prevent similar mistakes in the cockpit, you only employ JDAMS on SPIs with a steerpoint as the SPI generator, and you never (ever) create steerpoints on known friendly positions.
Maybe there's something in the audio that points to something else. If so, my apologies.

But like you said, most likely an error in the 9-line not noticed in the read back.
Also shows the value of a (secondary) mark.

I read something just yesterday about a similar incident in which the TACP replaced the battery in a GPS unit, causing it to display its own coordinates instead of where it was pointing and resulting in the same kind of friendly fire mishap.
