Seeing as the input itself is registered it has to be something within the game, not a matter of input device/mapping.

FOUND IT! Pfff...
I have a virtual joystick device installed for purpose I won't go into in order to keep it brief. It seems that removing this device fixes the issue. Actually pure luck I stumbled upon this because I totally ruled out controllers/mapping as DCS' controls indicator recognized my actions and so did the trim knob in the virtual pit.

Once upon a time not all that long ago I could coax my profiler to dump the entire set of variables and values. Maybe over-kill, but "hidden variables" were there to be found.
With key bindings ... those are all in RAM somewhere ...
(Trouble-shooting / de-bugging / Fault Isolation and Identification ... for me it's all like finding and neutralizing a sniper.)
